An accountability worksheet records a promise, the evidence that would show completion, and what you will do if the promise changes. A confession without a repair decision leaves the practical problem intact.

Completed fictional example — SAMPLE DATA
This example shows how to fill in the form. It is not a real person’s record, a diagnosis, a validated assessment or a promised outcome. Use your own observations and appropriate professional advice for actual health concerns.

A worked situation: Send the June newsletter
Fictional example. A volunteer promised a newsletter Tuesday, sent nothing, and avoided the group chat. An honest record separates the missed email from the unsupported claim that everyone now dislikes her.
Turn a missed promise into a repair decision
In the newsletter example, "draft completed" and "email delivered" are different states. Keep the original Tuesday promise in the record, then name who must approve the late delivery. A revised Wednesday date does not erase the missed commitment. The useful output is one accurate update and an authorized next step, not a confession about being an unreliable person.
