An interoception body-check chart distinguishes a noticed sensation, your interpretation, and the next ordinary need or question. A blank entry is valid. Not noticing a sensation does not establish a disorder.

A worked situation: Abdominal pressure
Fictional example. A reader writes pressure in the abdomen, then realizes hungry was an interpretation rather than a direct observation. She records the last meal as context and leaves the cause uncertain.
Use the chart to separate observation from interpretation
Write a sensation in one cell and the explanation you are considering in another. "Hungry before lunch" and "my signals are defective" are different kinds of statement. The chart cannot validate perception accuracy or establish a cause. Leave a signal unclear when it is unclear, and stop the record if it encourages escalating checking instead of an appropriate next decision.
