A perfectionism worksheet tests one quality rule against a defined recipient and purpose. Include a field for a genuinely essential check, so the experiment cannot be used as a reason to release inaccurate or unsafe work.
Work through these questions: The completed gallery
Completed fictional example — SAMPLE DATA
This example shows how to fill in the form. It is not a real person’s record, a diagnosis, a validated assessment or a promised outcome. Use your own observations and appropriate professional advice for actual health concerns.
| Question or field | Your entry |
|---|---|
| Recipient need? | A readable event announcement |
| Essential check? | Correct date and working registration link |
| Extra rule? | A personally imposed third layout revision |
| Release limit? | One fact check and one readability pass |
| Feedback plan? | Ask whether any essential information was missing |
Blank worksheet — copy or print
Copy the form into your own private document or print this page. Replace each “Your entry” cell with your own note; leave anything uncertain clearly marked as unknown. This page does not save or submit your entries.

| Question or field | Fictional completed example |
|---|---|
| Recipient need? | The completed gallery |
| Essential check? | Promised images present |
| Extra rule? | One perfect sequence |
| Release limit? | One final completeness check |
| Feedback plan? | Invite specific correction requests |
A worked situation: The completed gallery
Fictional example. A photographer keeps reordering a small client gallery after all promised images are present and corrected. The recipient needs the delivered gallery, not an endlessly reopened sequence.
Complete one sheet for one release
Write the recipient’s actual need before listing your extra requirements. Identify the check that protects that need and the point at which the item can be released. In the sample, another decorative revision should not silently become mandatory. If a real defect appears, record it and correct it; the stopping rule is not permission to bypass necessary quality or safety checks.
Another release case: financial checks before decorative revision

|---|---|
| Task? | Send the financial summary |
| Essential check? | Totals match source records |
| Avoided release? | Emailing the client |
| Optional revision? | Change the color theme |
| Bounded action? | Verify totals and send the qualified draft |
A worked situation: Send the financial summary
Fictional example. A freelancer rebuilds a spreadsheet's color theme instead of sending correct figures. One wrong total would matter; the third shade of blue does not resolve that risk.
