A symptom tracker should help communicate onset, duration, context, and daily impact without continuously searching for abnormalities. Keep uncertain estimates visible. The tracker organizes history and cannot establish a diagnosis or cause.
Work through these questions: Tiredness in my own words
Completed fictional example — SAMPLE DATA
This example shows how to fill in the form. It is not a real person’s record, a diagnosis, a validated assessment or a promised outcome. Use your own observations and appropriate professional advice for actual health concerns.

A worked situation: Tiredness in my own words
Fictional example. A reader records a recurring symptom once after an episode. She includes what ordinary task became difficult, rather than repeatedly measuring it until she feels certain.
Use one entry per concern or meaningful change
Record a plain-language description, approximate onset, context, and effect on ordinary activities. Mark an unknown date as approximate. Bring the record and questions to the appointment instead of expecting the form to identify the cause. Do not delay appropriate care to collect more entries, and do not make checking every sensation a condition of feeling safe.
