A useful habit-tracking sheet records the behavior you chose, not an invented score of personal worth. Keep the mark precise enough that a future review can distinguish an attempt, a completed action, and a planned rest day.

Work through these questions: Practice bars 12–16 once slowly

Completed fictional example — SAMPLE DATA

This example shows how to fill in the form. It is not a real person’s record, a diagnosis, a validated assessment or a promised outcome. Use your own observations and appropriate professional advice for actual health concerns.

Question or fieldFictional completed example
Behavior definition?Practice bars 12–16 once slowly
Cue?After setting down the work bag
Evidence?Date and bar practiced
Rest rule?Sunday planned off
Review question? Was the cue workable this week

A worked situation: Practice bars 12–16 once slowly

Fictional example. A musician writes yes after opening her instrument case, although her commitment was to practice one difficult bar. The sheet needs a completion rule before it needs more colorful boxes.

Use entries that can be checked

For a reading habit, "read" is too vague if you later cannot tell what counted. Write "read one agreed section and note one question." Leave planned rest distinguishable from a missed session. At review, ask whether the record changes your next decision; remove fields that merely produce a longer streak display.
